Use your code books and write your reasoning before opening the walkthrough. This exercise contains no real patient information.
Operative purpose
Synthetic adult is admitted for resection of a right frontal intracranial mass. Preoperative imaging localizes the lesion beneath the planned right frontal access. No earlier skull defect or separately necessary skull reconstruction is documented.
Access and treatment
The surgeon creates a right frontal bone flap, opens the dura and excises the identified mass through that opening. The dura is closed. No second cranial operation or unrelated access site is described. Final histology is outside this procedure-inclusion exercise.
Closure
The same bone flap created at the beginning of this operation is returned to its original position and secured. No defect larger than the current flap is repaired. The fixation hardware is documented as part of completing the craniotomy.
Review issue
A draft adds cranioplasty because the note says the bone flap was replaced. Apply the NCCI distinction between routine replacement of the current flap and other qualifying defect repairs. Exact selection of the intracranial operation still requires its full operative details and authorized entry.
Your coding worksheet
- Identify when the skull defect was created.
- Compare the repaired area with the current flap.
- Evaluate the additional cranioplasty proposal.
- State what different history would require another review.
In your notebook, record the supported facts, your index route, the instructions you checked, and any missing detail that limits your answer.
Compare with the worked explanation
- The defect was created for the current craniotomy.
- Only the current flap is replaced; no older or larger defect is supplied.
- NCCI C.4 includes this flap replacement in the craniotomy.
- A prior-encounter defect or a larger repaired defect would require its own complete factual and entry review, not an assumption based on fixation hardware.
