Follow-up context

A fictional adult returns to the orthopedic practice after treatment of a left forearm fracture. The practice applied the original cast during the earlier treatment encounter and retains the application record.

Current assessment

The clinician evaluates healing and authorizes cast removal. A trained staff member removes the cast under the practice’s usual workflow. This employee did not personally apply the original cast, but both employees work for the same practice entity.

Device record

The note describes removal only. It does not document a replacement cast, a separate repair of the device or treatment at an unrelated site. Any evaluation or rehabilitation work would require its own documentation and applicable global-period review.

Proposed billing

A proposed cast-removal line is justified solely by the fact that the employee differs from the original applicator. The learner must decide whether that personnel change satisfies the NCCI different-entity condition.
