Injury record
A fictional adult presents after a fall onto an outstretched left hand. Imaging and the treating clinician document a displaced extra-articular fracture of the distal left radius. The skin is intact, the injury is documented as closed, and no associated ulnar fracture is diagnosed. This encounter provides active treatment.
Treatment report
The physician performs documented closed reduction with manipulation and confirms the result. A short-arm cast is applied to stabilize the treated fracture. The physician documents the post-treatment assessment and instructions and assumes the subsequent fracture care. The exact current treatment entry must be verified from the authorized book.
Included-work review
The same professional encounter includes the initial cast application to the same anatomical site. The proposed charge contains both fracture treatment and a separate cast-application procedure. Supply reporting is a separate question and is not established by merely adding an application code.
Learner record
Abstract the injury separately from the procedure. Do not change the diagnosis to an open fracture because reduction occurred, and do not omit the manipulation documented in the treatment note. The scored question focuses on the first-cast inclusion, while the worksheet asks for the broader lookup route.
