Clinical order

For a fictional adult being treated for hypoglycemia, the physician orders an initial laboratory glucose measurement at 09:00 and another at 10:00 to assess the response to treatment. Both are stipulated to be eligible clinical diagnostic laboratory services under the same reporting entry.

Collections and results

The valid 09:00 specimen yields glucose 48 mg/dL. After treatment, a new valid 10:00 specimen yields glucose 86 mg/dL. Each collection and result has its own timestamp; both results guide the patient’s care.

Quality record

The record identifies no specimen failure, equipment malfunction or rerun merely to confirm an initial result. The repeat is intended to obtain another clinically necessary result.
