Treatment course
Fictional adult continues outpatient therapy under an existing plan. The therapist reviews progress before the scheduled treatment and adjusts routine exercise performance within that plan. The note describes expected progress rather than a new clinical change requiring formal reevaluation.
Completed work
The therapist asks about symptoms, checks the response to the prior visit and proceeds with the planned intervention. The record contains no separately documented reevaluation prompted by a meaningful change in status. Routine reassessment supports the ongoing treatment.
Draft claim
The charge template automatically adds a reevaluation at each visit, including this one. A modifier is proposed to bypass the combination edit. The coder has no additional record establishing why a formal reevaluation was medically necessary.
Review task
Compare routine monitoring of progress with a separately reportable reevaluation. The learner should identify the missing clinical basis instead of treating a template line as evidence that the service occurred. An actual changed-status reevaluation would require its own documentation and applicable modifier review.
