Encounter

Synthetic adult attends one physician-office infusion encounter. An existing double-lumen central catheter enters one vascular site. No new catheter, second vascular access site or separate return encounter is used.

Administration

Two ordered nonchemotherapy drugs run simultaneously through the two lumens from 09:00 to 10:00. The nurse records separate drug names, doses and pump channels in the medication record. This exercise tests the access-site relationship; drug-specific eligibility and final administration entries require the complete medication record.

Claim proposal

The draft contains two initial infusion services, one for each lumen. The proposed justification is that two substances and two lumen labels appear in the record. No separately necessary second site or independently necessary return encounter is documented.

Review scope

Determine whether the documented access facts establish the separate-site exception. Then explain what additional information would be needed to select initial, subsequent or concurrent entries for the actual administrations. Do not infer two sites from two medication records.

The record supplies the office setting and simultaneous 09:00–10:00 intervals. Final entry selection still requires the actual drugs, primary treatment purpose and complete administration instructions.
