Use your code books and write your reasoning before opening the walkthrough. This exercise contains no real patient information.
Blood-bank record
Fictional outpatient receives a documented blood product. The product record identifies its type, unit and preparation. For this exercise, the selected current HCPCS product entry expressly includes irradiation; the learner must verify that inclusion in the complete entry.
Preparation record
The blood bank documents irradiation of that same product before release. The irradiation log and product label refer to the same supplied unit. No separate product or additional independently supplied irradiation service is described.
Billing scope
The claim review concerns the product and its irradiation only. Compatibility testing, administration and other transfusion work are outside this focused record and must not be inferred. Each would require its own performed-service and inclusion review.
Draft claim
The draft reports the inclusive product plus an irradiation procedure for its preparation. The coder must decide whether the separate log establishes another service or documents work already represented by the product entry.
Your coding worksheet
- Identify the product’s stated inclusion.
- Match the irradiation log to the same unit.
- Evaluate adding irradiation separately.
- Keep other transfusion services outside this narrow conclusion.
In your notebook, record the supported facts, your index route, the instructions you checked, and any missing detail that limits your answer.
Compare with the worked explanation
- The selected product includes irradiation.
- The preparation log describes that same product unit.
- NCCI J does not allow adding the included irradiation again.
- This finding does not decide unprovided compatibility or administration services.
