Fictional adult undergoes a planned left-eye ophthalmic operation requiring a corneal incision. There is no traumatic eye injury in the history for this encounter. The operative report records the incision as access for the planned procedure.

Operative course

The surgeon performs the planned intraocular work through that incision. No additional injury at another site is documented. The incision is then closed as part of completing the operation, with the closure method and tissue condition recorded.

Record distinction

The note uses the word repair in its closure paragraph, but the opening was created surgically during the same encounter. The terminology must be read in context. It does not establish a separate accidental laceration or a second operative encounter.

Claim audit

The proposed claim adds a traumatic corneal laceration-repair procedure to the main operation. Evaluate the proposal using the origin of the wound and NCCI’s surgical-incision rule. Keep the closure details in the clinical record even when the work is included.
