Device problem

A fictional adult has a documented problem involving a lead of an implanted cardiac device. The operative plan concerns lead repair. The record identifies the existing generator and does not state that it is being replaced with a new unit.

Operative work

The clinician opens the device pocket, temporarily removes the generator as needed to access the lead, repairs the lead and returns the original generator. The device identity before and after the procedure is the same.

Component record

The operative inventory confirms that no new generator is implanted. The report’s words removal and reinsertion describe handling the original device during the repair. They do not establish replacement with another generator.
