Referral and indication
Synthetic record, September 9, 2026. An adult with persistent cough and recurrent small-volume hemoptysis is referred for diagnostic bronchoscopy. The clinician documents a lower-airway evaluation as the purpose of this examination. There is no separate nasal or laryngeal mass requiring another endoscopic procedure.
Signed access report
A flexible bronchoscope is passed through the nasal cavity, pharynx and larynx into the trachea. The clinician records access-region findings and vocal cord appearance while advancing the scope. These observations are part of the route to the bronchial tree, not a separately indicated upper-airway examination.
Completed examination
The clinician inspects the trachea and bilateral bronchial tree and documents no visible endobronchial lesion or active bleeding. No biopsy, lavage, brushing or therapeutic intervention is performed. No separate laryngoscope is used. The examination is completed and follow-up of the symptoms is planned.
Procedure worksheet
A draft lists diagnostic bronchoscopy plus separate nasal endoscopy and laryngoscopy because those regions have detailed descriptions. Evaluate the extra lines under Medicare NCCI. Use the completed diagnostic bronchoscopy and full current entry; do not infer sampling or treatment from inspection alone.
Your coding worksheet
- Trace the access route and target.
- Identify whether independent upper-airway indications are supplied.
- Evaluate the proposed extra scope lines.
- Explain why complete access observations remain included.
In your notebook, record the supported facts, your index route, the instructions you checked, and any missing detail that limits your answer.
Compare with the worked explanation
- The upper-airway structures are traversed to reach the bronchial tree.
- No independently necessary procedure in those access regions is documented.
- NCCI includes the ordinary access-region evaluation in the bronchoscopy.
- A detailed description does not multiply services. Complete the actual bronchoscopy entry without inventing another scope or intervention.
